All pledge checks should be made payable to: CT BREAST HEALTH INITIATIVE, INC. (or CT BHI) and mailed to the address listed, below.

Where do I send pledge money collected?

1. All pledge forms must be mailed with a check(s) for the entire pledge amount in order to qualify for our special prizes.   If you used the online Personal Headquarters to track pledges, print a copy of the Pledge Summary Report and include it with the checks.
2. Combine all checks, total the amount and mail to:

3. Do not send cash!   Please convert your cash into either a money order or a check.
4. If registering by mail, you must include your registration fee as a separate payment.   Pledge money is not a substitute for your registration fee and, therefore, must be paid separately.
5. The Connecticut Breast Health Initiative is a not-for-profit organization with Section 501(c)(3) status for tax purposes, and is governed by a Board of Directors.   Donations are tax deductible to the full extent allowed by law.

PLEDGE MONEY IS ACCEPTED THROUGHOUT THE YEAR BUT IN ORDER TO BE ELIGIBLE FOR RACE INCENTIVE GIFTS ALL PLEDGE MONEY MUST BE TURNED IN AS FOLLOWS:

• Before the race by mail or as an online donation or on May 7 at the Friends Asking Friends Pledge Tent
• May 8, 2010 RACE DAY - visit the Pledge Tent to turn in your pledge money and pick up your special prize *
• June 30, 2010 - deadline for turning in pledge money in order to receive incentive gifts.
